Matria CHF Management reduces costly hospitalizations by helping employees understand the serious health effects of not adhering to their care plans, recognize symptoms that could worsen their condition or lead to other illnesses and modify unhealthy lifestyle behaviors regarding weight, diet and activity level.

Through educational materials and guidance from our nurses, we help patients take better care of their overall health to combat symptoms, such as fatigue, shortness of breath and swelling from fluid retention.
