Matria ESRD Management manages the care of patients with irreversible kidney failure who require dialysis treatments while they wait for a kidney transplant. Certified nurse case managers act as a liaison between patients, physicians, specialists and employers to coordinate, evaluate and monitor patient care and dialysis treatments.

Our goal is to help patients prevent medical complications that lead to unnecessary and costly hospitalizations, emergency room visits and doctor visits and reduce your overall healthcare costs.

• A small patient-nurse ratio guarantees that patients receive as much attention as they need.

• Matria’s ESRD nurses are certified case managers.
