Disease Management is rapidly being adopted by corporations, health plans and federal and state governments as a solution to rising healthcare costs. A proactive method for managing the health of a small but critically important group of employees, disease management programs target the 15 percent of employees who generate the greatest amount of medical costs - not the 85 percent who are healthy.
